SafeLabCentre.com is owned and operated by the Wright Agency LLC. All testing information obtained on your behalf by Wright Agency LLC is confidential and will not be shared with 3rd parties without your authorization. All testing results will be reviewed by our partner physicians then released for your review. Wright Agency LLC and its partners are required by law to report positive test results for all communicable diseases. In these cases we only report the minimum amount of information required by law to your local State Department of Public Health. We will never report any personal information or test results to your health insurance company or any other individuals or entities without your authorization. Confidential STD Testing
SafeLabCentre.com services and materials are provided by Wright Agency LLC, and results provided to you by Wright Agency LLC are for informational purposes only. Wright Agency LLC employs a national network of qualified physicians that prepare the test requisitions and review results. In the event of a denied test request, Wright Agency LLC will refund your fees in full. Our counselors are not doctors and do not diagnose or treat medical conditions or give medical advice. Getting tested through SafeLabCentre.com does not replace the care of a medical practitioner or counselor. Wright Agency LLC is not responsible for any pre or post testing treatment or health care. In the event of a cancellation your testing fees will be refunded minus a $50 cancellation fee if the testing service is cancelled before testing is done, and within 14 days of scheduling. Payment plans are available; results will be made available when account balances are paid in full. By arranging testing with Wright Agency LLC you understand and agree to the following statements. I am 18 years of age or older and I am the person being tested. I will provide a blood/urine sample at a collection site. I am not being tested for every sexually transmitted disease. If my test results are positive Wright Agency LLC and it's partners are required by law to provide my name, address, and results to my State’s Department of Public Health. By law, the State Department of Public Health cannot compromise my confidentiality. I am responsible for calling SafeLabCentre.com back for post-test counseling and results. While the panel of STD tests are highly specific and highly sensitive, there is a small possibility of a false positive or false negative result. SafeLabCentre.com counselors do not provide medical advice or recommend treatment. I release and hold Wright Agency LLC and it's partners harmless from any injury or personal damage that occurs while I’m on PSC premises. Patient Authorization for Use and Disclosure of Protected Health Information
I authorize my health care providers, including Physicians Wellness Network, its physicians and its staff and the laboratories that run medical tests for me to use and/or disclose certain protected health information about me to Wright Agency LLC and its affiliates for the purposes stated below. This authorization applies to the following protected health information about me: the laboratory requisition submitted by me and the laboratory test values which are the result of the laboratory test(s) requested in the requisition. For avoidance of doubt, I specifically authorize the transfer of this information between and among myself and the following individuals, organizations and their representatives: i) Wright Agency LLC, ii) Physicians Wellness Network staff, iii) the designated Physicians Wellness Network physician of record, and iv) the reference laboratory of record. I understand that the Physicians Wellness Network physician of record may be required to receive my lab test results before I do, and that this physician’s authorization to release those results to me may also be required before I receive my results. The protected health information will be used or disclosed for the sole purpose of complying with the state and federal laws which may require a physician or their agent to: 1.) review and approve a laboratory requisition; and 2.) review the laboratory test results. This review may be conducted for any reason, including in the event laboratory values, which are outside of normal ranges, require the physician or its agent to contact me. The purposes outlined above are provided so that I can make an informed decision whether to allow release of the information to the parties referenced in this authorization. This authorization will expire one year after the date of this authorization. Physicians Wellness Network and its designated physician of record may receive payment or other remuneration from a third party in exchange for reviewing the protected health information. I understand that I have a right to receive a copy of this authorization. I understand that the sole purpose of the laboratory test is to generate the results of the tests that I have requested, and the laboratory tests will not be requisitioned unless I sign this authorization. I have the right to refuse to agree to this authorization. When my information is used or disclosed pursuant to this authorization, it may be subject to re-disclosure by the recipient and may no longer be protected by the federal HIPAA Privacy Rule. I have the right to revoke this authorization in writing at any time, except that the revocation will not apply to any information already disclosed by the parties referenced in this authorization have acted in reliance upon this authorization.
